Most likely someone raised the tipper without making sure all was clear.tractor was probably on the way off when it raised, the steep angle could cause fuel and oil leaks with tractor still hooked.
Im going with "operator error" Which operator is up to you... I would say that this truck driver didnt want to take his coffee out of the cup holder and unhooked his truck from the trailer and was driving off when the lift operator started the lift up...

You are right that they do tip with tractors hooked, but some companies will refuse the load if you don't drop the trailer because they won't chance the environmental liability. I have seen fuel leak with them in air.
