as a former trucker, i suspect both of these happened when they were sliding the trailer axles to balance the weight and forgot to lock them back. the trailer axles are on a track and can slide on the track to shift the weight, and if you slide them and forget to walk back and lock the pins, then drive down the road and hit the brakes, this is what happens, i did it once, except the axles didnt come totally off the track,
