The pileup, which killed a Canadian truck driver and injured 18 others, involved a fireworks truck and a formic acid tanker. The report named the portion of I-94 where the pileup occurred the most dangerous highway in the state.
According to the report, the cable barriers constrained the flow of traffic after the pileup began and caused more vehicles to become involved in the crash.
